Elevator Accident Reporting

Oregon Revised Statute 460.045(7) prohibits a person from placing an elevator back in service, if the elevator has caused an injury to a person. If the elevator caused an injury to a person, report the accident to the Building Codes Division and obtain permission from the Chief Elevator Inspector before reactivating the elevator.

An Elevator Accident Report must be filed with the division.

What to do in the event of an elevator accident:

Determine the cause of the accident through the use of a licensed elevator contractor.

  • If the elevator did not cause the accident, the licensed elevator contractor may reactivate the elevator without notifying the division.

  • If the elevator did cause the accident, report the accident to the Chief Elevator Inspector and file an Elevator Accident Report with the division. After repairing the elevator, obtain permission to reactivate the elevator from the Chief Elevator Inspector.

This process does not supersede any other law or rule requirements such as reporting to the appropriate insurer or OR-OSHA in the event of a work place injury.
